For a mat or pad specifically designed for skin contact, the ideal distance is usually direct contact---or the small gap stated in its instructions---not a panel-style stand-off distance. The session time should be based on the device's documented irradiance at the skin surface. If that information is missing, do not try to create a precise dose estimate from electrical wattage, LED count, or rules meant for rigid panels.

Should You Lie Directly on a Red Light Mat or Leave a Gap?

Start with the device's intended treatment geometry.

A flexible mat, pad, wrap, or wearable device designed for contact use is built around a fixed relationship between the LEDs and the body. In that case, distance is not usually something to optimize independently: the intended position is direct skin contact or the specific small separation described in the instructions.

A rigid panel is different. Panel guidance often discusses distances such as 10--30 cm because a panel's irradiance changes substantially as the user moves farther away. That range is not a default rule for a mat or pad resting on the body.

Use this practical split:

  • Contact-designed mat or pad: Use it against bare skin if the instructions permit contact and the position is comfortable.
  • Mat or pad with a specified gap, cover, or clothing layer: Follow that documented setup rather than assuming bare-skin contact is appropriate.
  • Non-contact device used while lying down: Use its stated treatment distance. Do not treat it as a contact mat simply because it is flexible or placed on a bed.

The key question is not, "How close can I get?" It is, "At what position was this device designed and specified to operate?"

Why Distance Matters Even When You are Lying Still

Distance affects irradiance, the light intensity reaching the skin per unit area. Irradiance is expressed in milliwatts per square centimetre, written as mW/cm².

If session time stays the same, lower irradiance means a lower delivered energy dose. A gap between a mat and the body can therefore reduce dose compared with direct contact---unless the device's specifications were measured and intended for that gap.

For panels, moving farther away generally lowers irradiance. A simple inverse-square approximation is sometimes used for light spreading from a relatively concentrated source: doubling the distance may reduce irradiance to roughly one quarter. But that shortcut should not be used to calculate output from a broad, flexible, body-conforming mat. Its LED layout, optical design, bend, and contact pattern can make the actual exposure geometry more complex.

For a mat, the useful number is therefore not a generic distance rule. It is:

  • irradiance measured or specified at the contact surface, for a contact device; or
  • irradiance specified at the device's stated treatment distance, for a non-contact setup.

An irradiance number without a stated measurement position is incomplete. Two devices can report very different irradiance at the same distance, and a device's electrical wattage does not tell you how much light reaches the skin.

Do Clothing, Sheets, and Covers Change the Dose?

Yes. Any material between the LEDs and the skin can reduce the light that reaches the body.

Bare-skin exposure avoids the attenuation introduced by clothing. Textile transmission varies with fabric type, color, weave, thickness, number of layers, and wavelength. A thin garment, thick clothing, bedsheet, or built-in cover cannot be assumed to reduce light by one universal percentage.

That means a contact-surface irradiance value is only directly useful when your setup matches the measurement conditions.

For example:

  • If a mat's irradiance was documented at direct skin contact, that value may not apply through clothing or a sheet.
  • If a cover is part of the documented intended-use design, use the device's instructions rather than assuming it invalidates the specification.
  • Multiple clothing layers are more likely to reduce transmission than a single layer, but the amount cannot be calculated reliably without material- and wavelength-specific data.

How Do You Calculate Dose From Irradiance and Time?

The delivered energy dose is often called fluence and is expressed in joules per square centimetre, written as J/cm².

You can calculate it only when irradiance is known at the actual skin-to-device geometry. In plain terms:

  1. Take the documented irradiance in mW/cm².
  2. Multiply it by session length in seconds.
  3. Divide the result by 1,000.
  4. The result is the estimated dose in J/cm².

A Hypothetical Calculation

Suppose a device is documented to provide 50 mW/cm² at the skin, and the session lasts 10 minutes.

Step Calculation
Convert 10 minutes to seconds 600 seconds
Multiply irradiance by time 50 × 600 = 30,000
Divide by 1,000 30 J/cm²

That session would deliver an estimated 30 J/cm² under those conditions.

This is a math example, not a universal target. It assumes the stated 50 mW/cm² is accurate at the skin for the full session and that the device maintains the same output throughout use.

Do not treat the result as precise when any of the following are unknown:

  • The irradiance measurement distance or contact condition
  • Whether a sheet, clothing layer, or cover was present during testing
  • Whether the mat has shifted or lost contact
  • How pulsing or duty cycle was handled in the published irradiance value
  • Whether the specification represents a measured skin-surface value rather than a marketing output claim

How Long and How Often Should You Use a Mat?

There is no universal session length or frequency schedule that fits every red or near-infrared mat.

The appropriate time depends on the device's skin-surface irradiance, wavelengths, treatment area, intended use, and the dose the instructions specify. A lower-irradiance device may require more time to deliver the same calculated dose as a higher-irradiance device, but that does not mean longer is automatically better.

Photobiomodulation research describes a biphasic dose response: in some settings, lower exposures may be more useful than higher exposures, and the optimal dose can vary by application and parameters. For practical setup:

  • Use the session duration and frequency stated for your specific mat or pad when those directions are available.
  • Do not extend sessions simply because the device is in direct contact.

  • If you calculate a dose, use it as a check on documented instructions---not as a substitute for them.

Do Red and Near-Infrared Light Need Different Distances?

Red light is commonly described for more superficial targets, while near-infrared light is often selected for deeper targets. But those differences do not establish separate contact-distance rules for a mat designed to rest against the body.

Both wavelengths still require the same basic technical information:

  • Which wavelengths the device emits
  • Irradiance at the skin surface or specified gap
  • Session duration
  • Treatment area
  • Intended contact or non-contact position

The device's documented specifications, not perceived brightness, should guide session setup.

What If the Mat Feels Too Warm or Causes Redness?

Warmth or redness can be a sign that the exposure is too intense for that setup, particularly with higher-irradiance devices. It does not by itself prove injury, and contact mats should not automatically be treated like high-output panels. Still, discomfort is useful feedback.

Stop or shorten the session if warmth becomes notable, the skin becomes uncomfortably red, or the treatment position is not tolerable. Then return to the instructions for the device's permitted session length, positioning, and heat-related precautions.

Do not solve a heat problem by inventing a random gap if the mat was designed for contact. A gap changes irradiance and dose. Instead, use only the positioning options the instructions allow, and reduce exposure time when appropriate under those directions.

A Specification Checklist Before You Lie Down

Before relying on a mat or pad for a calculated dose, check whether its documentation provides:

  • Red and near-infrared wavelengths, if applicable
  • Irradiance in mW/cm²
  • The exact distance or contact condition used for that irradiance value
  • Treatment area or coverage information
  • Recommended session duration and frequency
  • Intended positioning: direct contact, a cover, a gap, or clothing
  • Heat, comfort, and skin-response precautions

The practical decision rule is simple: use the position specified for that mat or pad, prioritize bare-skin contact only when the instructions support it and it is comfortable, and base session time on documented skin-surface irradiance---not panel-distance rules, wattage, or LED count. If the technical specifications are incomplete, treat that as a reason to avoid false precision and rely on the documented use instructions instead.
